SUPPORTING DOCUMENTS FORM

In this final stage of the application process, you will need to upload a range of documents to support your application. Some of these are mandatory documents, while others are optional and you can use the spare upload boxes at the bottom of this form to add any additional items that you feel are necessary to support your case.

Some of the documents you will need to sign and upload are available to download on our useful resources page, along with a range of other free templates that you can use at your discretion.

IMPORTANT INSTRUCTIONS BEFORE YOU GET STARTED:

> Saving your form: 

Unless you’re ready to insert all of your information in one go and submit this form immediately, you may want to save your form so you can return to it at a later stage.

Please do this BEFORE you start completing the form, using the “Save OR Return to my application” button below. 

Once this has been completed, you will see a confirmation at the top of your form – “Logged in as [*your username*]” – and a “Save my progress” button at the end of your form.

You can then use the “Save OR Return to my application” in future, signing in as a “returning user” with your username and password.

> Creating your username: 

You will need to create a NEW username, rather than re-using your username from Stage 1 and 2, in order to correctly save this new form, however you can still use the same email address.

You may like to add a ‘3’ to your previous username for ease.
